If you have an upcoming appointment, be sure to complete your medical history form on line.  This may take you 10-15 minutes now, but will save time during your office visit, not only for yourself but for our office staff!

If you have already been to the office and completed the “bubble sheet” medical history form, you do not have to complete this again. If you have provided us with your e-mail address, you will be able to sign-up to have access to your medical history—so please take a few moments to review this information before your appointment.  Let us know if your information has changed and anything needs to be updated.

If you have not given us an e-mail address, call our office at the phone number below so we can add this to your account.  Once this is done, you will then be able to click the sign-up link on the right to register and have access to your information.
